Where the official schedule gives an age window (12 to 15 months, say) the table places the visit at the start of the window, so a reminder arrives before it closes rather than after. Booster doses appear at the age they are given, not next to the first dose.
Usually not. An interrupted series is completed from where it stopped, on a catch-up plan the clinic sets. What matters is not to delay the catch-up: the younger the child, the more a gap without protection counts. Note what has been done and bring the record to the next visit.
